What is Biceps Tendonitis?

Biceps tendonitis is a condition in which the main tendon connecting the shoulder to the upper arm becomes inflamed, typically due to overexertion, wear and tear, or injury. This can cause pain in the front of your shoulder, as well as in the affected arm. In addition, patients with this condition typically experience weakness in the shoulder and arm, and other symptoms.


These types of conditions are commonly seen among athletes, as well as those who regularly lift weights or perform other rigorous upper body exercises. Because the biceps muscle is responsible for aiding in shoulder stability, any injury or disability in this tendon can severely impact physical activity. In the severest cases, surgery may be required to repair the impacted tendon. In most instances, however, noninvasive osteopathic treatment can be an effective way to treat this type of tendonitis.

What Causes this Condition?

Your bicep muscles and tendons are used in many ways each day. As a result, it is not uncommon to overexert the tendons, leading to aches and soreness caused by micro tears in the tissue. However, there are a variety of reasons why this tendonitis may occur, including:

  • Repetitive motions, such as arm and shoulder movements common in many sports
  • Sudden overload of the tendon
  • Physical injuries, like the trauma experienced in an automobile accident or fall
  • Damage to this tendon can occur at either end of the biceps, leading to pain at either the shoulder or elbow.

When tendon tears occur near the shoulder, they are referred to as proximal biceps tears. Tears near the elbow are called distal biceps tears. The latter are less common and occur primarily due to injuries.

What Are the Symptoms of Biceps Tendonitis?

This type of tendonitis is usually accompanied by sharp pain in either the upper arm region and shoulder or near the elbow. The location of the pain is typically dependent on the location of the injury. Other symptoms can include:

  • Loss of mobility, particularly in arm rotation
  • Weakness in the shoulder, arm, or elbow
  • Bruising at either end of the upper arm
  • Noticeable changes in the shape of the front of the bicep
